Springfield Vital Records - Birth/Death/Marriage

VITAL RECORDS LAW ACT 46- EFFECTIVE JULY 1, 2019

BIRTH AND DEATH RECORDS

  • Copies of records in vault are no longer available.  All certified and non-certified copies must be issued by town staff from the State of Vermont Vital Records Issuance Management System (VRIMS).
  • Application and ID are required to request a certified copy of a birth or death record. The application is "fillable," however, you will need to download and print it.
  • Availability of certified copies is limited to certain people/organizations, based on relationship to person named on document. Please see Vital Record Application for authorized relationship to the person named on the certificate.

MARRIAGE RECORDS

  • Certified and non-certified copies of marriage records are to be made by staff only.  No application is required.

REQUESTING CERTIFIED BIRTH AND DEATH RECORDS THRU THE MAIL

  • Completely fill out the Vital Record Application, sign it and send it along with a photo copy of the acceptable form of ID stated on the Application, along with a check in the amount of $10.00 payable to the Town of Springfield, at the address stated on the Application.  Include a return envelope stamped and addressed back to yourself.  We are able to process most applications within 48 hours.  
REQUESTING CERTIFIED BIRTH AND DEATH RECORDS IN PERSON AT THE TOWN CLERK'S OFFICE 
  • Town Clerk's Office is open 8-4:30 M-F to process your request.  Completely fill out the Vital Record Application, sign it and present it along with the acceptable form of ID stated on the Application, along with a cash or check in the amount of $10.00 payable to the Town of Springfield.  Vital Record Application Instruction Sheet.
  • If you are a homeless service provider, you will need to submit the Affidavit of Homeless Status form along with the Application.
